Request Form Templates
Request forms formalise an ask so the approver has everything needed to decide without follow-up questions, reducing back-and-forth and creating a record of the decision. Templates in this collection cover the common contexts including time off and leave requests for vacation, sick days, or personal time, equipment and supply requests for office or field work, software access requests for IT approval, expense and reimbursement requests, refund and return requests for consumer transactions, and service modification requests for accounts and subscriptions. Each one includes the requester’s identification, the specific request, the business or personal reason, and the date by which the decision is needed. Pick the template closest to your situation and customise the approval routing.
Request forms work better when the requester thinks through what the approver will need to make the decision before submitting. A time-off request that includes coverage arrangements, current project status, and any deadlines affected is approved faster than one that asks for dates with no context. An equipment request that names the specific equipment, the business reason, the budget impact, and any alternatives considered gets a faster decision than one that simply names the item wanted. Pre-empting the questions the approver would otherwise need to ask reduces back-and-forth and increases the chance of approval.
For requests that involve approval chains (manager, finance, executive), submit through the proper routing rather than escalating directly, since skipping levels usually causes the request to bounce back to the right routing with the original delay plus whatever additional friction was created. For requests that involve coordination across departments, copy the affected groups on the request so everyone has visibility, even if their approval is not technically required. For repeated requests of the same type (recurring software subscriptions, regular time off patterns), consider whether a standing arrangement could replace the recurring form submission, since reducing administrative overhead frees both requester and approver for higher-value work.
